1. Invitation to Our Patient Portal
Our Patient portal is designed to enhance engagement with our patients by enabling you to update and access your electronic medical records (EMRs). This technology also facilitates secure patient-provider communication so we can best serve you. This is where you will provide all your pertinent medical information. You can even access your lab work, send us a message, or book an appointment for any of our aesthetic services via the portal.
2. Comprehensive Lab Testing
Once you are established in the patient portal we will send you for lab testing. Lab tests may be billed through your insurance company or discounted rates are available for self pay patients. The goal at CHS is to obtain comprehensive lab results prior to your appointment to help us identify the cause of your health concerns. Lab work is essential to uncovering exactly where any imbalance may lie.
3. Initial Consultation
At your first consultation, you will have a conversation with your provider about your health goals. Your practitioner will discuss all facets of your health as related to your current issues: physical symptoms, past medical history, diet, lifestyle, and mental/emotional stressors. Lab work will be reviewed in depth. A personalized plan will be discussed to help you improve your quality of life and reach your health goals. The initial consultation is one hour. Treatment plans may include nutritional modifications, supplement recommendations, lifestyle changes and plans for hormone balance.
4. Follow Up and Ongoing Care
The provider typically sees new patients 6-12 weeks after their initial consultation or until symptoms are resolved. Lab work is typically ordered 3-4 times per year. Your practitioner will determine which interval is appropriate for you. Follow up visits are 30 minutes. Along the way our team is always available to you as a resource for all of your personal wellness questions.

General Practice Information
What services do you offer?
We specialize in hormone replacement therapy (HRT), weight loss programs, and aesthetic treatments, including neurotoxin, dermal fillers, microneedling, VI Peels, and more.
Do you accept insurance?
We do not accept insurance but can provide itemized receipts for HSA/FSA reimbursement. Insurance can also be used for blood work.
Do you accept HSA/FSA cards?
Yes, we accept HSA and FSA payments for eligible services and products.
Can I return a product or get a refund on a service?
Due to the nature of our services and products, all sales are final. Please contact us if you have concerns about a purchase.
Appointments & Policies
What is your cancellation policy?
We require at least 48 hours’ notice for cancellations or rescheduling. A $50 fee will apply to late cancellations or reschedules. This fee must be paid before booking future appointments. Missing an appointment without notice incurs a $100 fee, charged at the time of the missed appointment.
What happens if I am late?
Patients arriving more than 10 minutes late must reschedule and pay the $50 late cancellation fee before booking a new appointment.
How do I schedule an appointment?
Appointments can be scheduled by calling our office or booking online through our website. All cosmetic services, including Botox/Dysport, Restylane/Juvederm fillers, Skinvive, SkinPen microneedling, VI Peels, dermaplaning, ear piercings, skincare/aesthetic consultations, and B12 injections, can be booked via the patient portal. Wellness and hormone consultations, as well as follow-up appointments, must be scheduled by contacting our office via phone or email.
Hormone Therapy & Treatment
What is hormone replacement therapy (HRT)?
HRT is a treatment that helps balance hormone levels, alleviating symptoms of menopause, perimenopause, and other hormone deficiencies.
How do I know if I need HRT?
Symptoms such as fatigue, weight gain, mood swings, hot flashes, and sleep disturbances may indicate a hormone imbalance. A consultation and lab work will help determine if HRT is right for you.
How long does it take to see results from HRT?
Most patients notice improvements within a few weeks, but optimal results can take a few months.
Do you offer treatment for men as well?
Yes, we offer hormone therapy for both men and women.
Weight Loss Services
What weight loss medications do you offer?
We offer compounded weight loss medications, including GLP-1 receptor agonists. Your provider will determine the best option for you.
Can I continue weight loss medication if I’m having surgery?
Patients must notify their provider if they have an upcoming surgery or procedure to determine whether medication adjustments are necessary.
How does your compounded weight loss program work?
Our program includes a customized medication plan, lifestyle guidance, and provider oversight to support long-term weight management. Patients participating in our weight loss program are required to complete annual lab work and attend in-office follow-ups every 6-8 weeks, depending on their treatment plan.
